Medical Assistants
Karina M. and Raquel R. have been dedicated to working with Dr. Chai for many years. Karina (pictured left) manages the back office, while Raquel handles the front office and their work synergy makes the appointments run very smoothly from patient to patient. When asked about their typical workday and goals, both agreed that they were very passionate about women’s healthcare and strongly believe in providing high-quality compassionate patient care. “At the end of the day, it’s about our patients’ overall well-being. ” said Raquel.
Insurance Policy
Effective January 1, 2021, Dr. Chai will be an out of network physician for all PPO insurances with the exception of Medicare. You can submit the super bill/encounter form to your insurance if you have out of network benefits. Also, please note that all tests and labs ordered (for example mammograms, PAPs, etc.) and prescriptions can still be billed directly through your insurance.
Patients should verify that Dr. Chai is a preferred provider for their plan prior to their visit. We are not a participating provider for any HMO plans, Anthem Blue Cross, Health Net, or Covered California.
Payment for services will be due on the day of your appointment. This only applies to your office visit and does not include labs, radiology, or prescriptions. For patients who have out-of-network benefits, you may wish to submit your claim to your insurance. Many PPOs will pay a percentage of charges and reimburse you directly.
A typical routine annual exam will be approximately $225-$275 that is stratified based on age.
If you opt to see a new doctor, I will provide routine refill requests for non-controlled medications for an additional three months until you establish care.
